Determining the Names of Children Who Are of Age

Please carefully complete the statement for a child’s use of a name (naming, initial determination for a person of legal age) using a typewriter or print. Signthe statement before anotary public, who must certify your signature.  If you come to the German foreign mission responsible for your district in person, your signature can be certified by a consular officer for a fee of €20.00, payable in US dollars based on the current exchange rate or by credit card.

 

Please do not fill out the forms using capital letters only, in some cases this can result in different spellings.

Submit the statement, along with the following documents, to the German foreign mission responsible for your district:

  • Certified copy of your birth certificate. If only the county is entered as the place of birth and not the city/town, you will also need the proof of birth letter from the hospital.
  • Certified copy of your passport
  • Certified copies of your parents’ passports
  • Certified copy of your parents’ marriage certificate (the Registry Office I in Berlin requires the marriage certificate issued by the county clerk)

Your German foreign mission will forward the statement and documents to the registrar in Germany, who will confirm the use of the name and, if ordered, will issue a certificate.
